If you would like us to share your information with anyone or any organization, you must contact us and follow our protocol.
Appointment Etiquette & Cancellation Policy
We would ask for $100 payment for booking appointment. All this amount will be applied to your services. Should you need to reschedule your appointment, please be sure to provide at least 48 business hours’ notice to avoid a cancellation fee of $100. Thank you for your understanding, as we allocate ample time and resources for your visit.
To ensure that all of the clients receive full attention, please plan to arrive approximately 15 minutes before your scheduled time. If you are running late, we appreciate a phone call. Your consultation is very thorough. Expect to spend up to an hour with us for your initial consultation.
Photograph Consent and Release
We take a profile picture and also take photos of you for our or parts of your body before and after procedure as part of your medical records. surgery. The photographs will be taken by one of the members of the Elite Medical staff.
Medical Care: Photographs, clips and videos taken of me or parts of my body can be used for the purpose of personal profile, my medical care with Elite Medical Specialty Clinic, The photographs and all details regarding medical services rendered to me will be kept within my personal medical history file at Elite Medical Specialty Clinic.
By signing this form, I acknowledge my consent as initialed above, and I further recognize that this consent form will supersede any other photo consent forms with a date prior to the date written.
In some cases we may want to use your photos on our website or our social medical. But If we plan to use your photos for social media and our website then we will ask for your permission.
Pregnant or Nursing Women
Due to medical grade treatments and formulations, some are not recommended during breast feeding and pregnancy.
Therefore if you think you may be pregnant or nursing then you must notify our staff at the time of scheduling appointment and at the time of visit. At Elite Medical Aesthetics, Dr. Ray would make recommendations of any possible options for you.

We encourage our clients to contact us through the website by submitting the contact form. Our staff may be able to respond out of the office hours too.
Initial Consultation
Initial Consultation may be complimentary. At Elite Medical Aesthetics of Rocklin, Dr Ray would like to offer an opportunity to you to have a comprehensive cosmetic consultation and to have a better understanding of the procedures and treatments for your specific needs.
Scheduling
We recommend scheduling your next appointment prior to leaving the medical spa.
Payment
Payment is required in full at the time of service for medical aesthetic services and products.
Advanced payment may be required on aesthetic treatments & procedures.
All major credit cards accepted and financing is available through third party including carecardit or other companies.
